Co-pays and any additional balances are to be paid at the time of your appointment. 

Medicaid plans are not accepted. Patients without insurance will be required to pay out of pocket.

It is the patient's responsibility to make sure that their plan is in network with our office by calling the number on the back of their insurance card. 

Any balances acquired because the patient's plan is not in network are the patient's responsibility and will be billed directly to the address on file for the patient.
